Case management is a vital service provided by
Northwest in all 5 counties we serve. These case managers or service
coordinators serve as advocates for individuals and families in
accessing and increasing services. They meet with the individuals
to determine what services are needed and then strive to help that
need. They continue to assist the family by monitoring the services
to insure they continue to recieve needed services and also to work
with them on any future needs. Currently seven case managers serve
approximately 180 individuals in our catchment area.
